We have a new record keeping system and we ask that new parents to OTM (and
continuing parents if their contact details change) fill and return an enrollment/booking
form. This is so our new system has your correct current details.
We MUCH prefer that you send us your completed enrollment/booking form electronically
(Email).

Please note we now ask you to provide several new bits of information about
yourself and your child, including emergency/medical practitioner contact details.
The form is designed so that you only need to fill in the one form for up to
three students in the same family. If you happen to have more than three children
enrolling at OTM, you’ll have to use two of the form.
On receipt of your completed booking /enrollment form, we will book a place
for your child/ren on the waiting list for the chosen class/es for that year
and send an invoice for the term.
Enrollment in OTM dance classes is on a per term basis, so as soon as the
term fee is paid, your child/ren will be enrolled and entitled to attend the
term’s
classes. Enrollment is on a ‘first come first serve’ basis. If
you do not enroll for a term we cannot guarantee a place for your child (some
classes have waiting lists).
Once enrolled in a particular class/es, you will only need to complete further
enrollment forms when you or your child/ren’s details change. For most,
this will mean completing just the initial enrollment form at the start of
every school year, ie, unless we hear otherwise from you, we will assume that
the booked classes and other details continue throughout the school year.
Fees are due no later than seven days before term starts.
